Self-pay / Uninsured Patients

A $265 new patient fee shall be collected on your first visit as a new patient before services are rendered. This fee includes the office visit and *baseline labs only. Follow-up fee is $95 per visit excluding diganostic and laboratory tests. Self-pay patients automatically qualify for special consideration in accordance with our financial assistance program. In-house laboratory tests and other procedures are already at discounted rates and are priced much less than laboratories and diagnostics centers. Please see list of our tests below:
