Insured Patients

We will bill your visit directly to your insurance provider if you supply an updated proof of insurance at each visit. Please note that the insurance policy is between you and your insurance provider, we are happy to file the claim for you. Copays, co-insurances are the patient's responsibility and any amount not covered by your insurance policy. Balances are due immediately after receipt of your first invoice.

MEDICARE & TRICARE
We accept patients on the Medicare & Tricare plans and will also bill your secondary insurance provider directly if any. Patients will be asked to sign an Advanced Beneficiary Notice for services not covered under Medicare. Patients shall be accountable for covering these services if the secondary insurance does not pay.

As a courtesy and for the convenience of our patients, we offer in-office blood draw service and other diagnostic tests and procedures.
