In order to make sure your visit to the practice goes smoothly, we've put together all the important information about how things work at the practice.
Don't forget to bring your insurance information with you
In order for us to register and treat you promptly, we request that NEW patients bring their current insurance ID, driver's license or photo ID on their first visit, together with the completed new patient registration form.
Please also bring your list of medication, allergy list and most recent laboratory and diagnostic results if any on your initial visit to the clinic.
Established patients are requested to present their updated insurance ID, and inform front desk of changes in their address and contact information during check-in for follow-up visits to ensure insurance and demographic information are up-to-date.
We accept most types of health insurance. If you do not have insurance, please ask us to learn more about our self-pay rates.
Company Policies
We do not practice psychiatry and pain management. Patients will be referred to psychiatry & pain management clinics/physicians for their specific condition.
If you are taking scheduled drugs, you will be asked to do a urine drug screening test before seeing the physician.
Presciption refills are only authorized for patients seen within the last three months. If you have not seen the physician for more than three months, you will be asked to consult the doctor for an updated evaluation of your medical condition and prescriptions before a refill is authorized. Please allow the office 48-72 hours to attend to your refill requests.
If you owe us a balance for more than 30 days, we will ask that you pay the balance in whole or a portion of it before you are seen by the physician. If you are in financial hardship, please ask us regarding our financial assistance payment program.
In consideration of the physician's time and other patients' time as well, beginning May 1, 2012, we will be charging NO SHOW FEES of $25 for patients who missed their appointment but failed to cancel at least 24 hours prior to their appointment.
